Healthcare forms can often be very nasty and psychologically difficult to handle: the design of many of these documents usually doesn't help, most of the time being extremely banal, trite, confusing and demeaning.

What fascinated me about this very broad category of forms is that they can literally cover an individual’s entire life span (and actually also go beyond it!): I, therefore, decided to collect a certain number of them, analyze them and design a series of health care related forms with different functions but similar visual features so that the feeling would almost be that of a common brand identity.

Healthcare related forms usually vary from state to state, from county to county or even from institution to institution: I tried to collect as many forms with the same function as possible and summarize the basic information that each one required, in order to create what I like to think of as "archetypal" healthcare forms.
